The Connected West Yorkshire - Death Certificate Flexible Data Model (FDM) Contains routinely collected data for approximately 2,131,922 patients. The dataset has been fully anonymised, but can link to other Connected West Yorkshire FDM"s. The Death Certificate FDM last build date was 2026-03-10 . The observation period for this data is:01/04/2013 to 11/02/2026 The FDM is made up of 1 source table using routinely collected data for 2,131,922 individuals, aged 0 to 100+ The tables are as supplied with minimal reformatting. It includes basic patient demographics, and information about consultations.

Access Service:
Expression of Interest (EOI) are reviewed 3 monthly by the Connected West Yorkshire Board. For the majority of EOI"s, a subset of various FDM"s fitting the requirements of the research will be provided for use by the researcher. The extract will only contain the variables required by the research. Access to data is via a secure R or Python environment hosted by the SDE. A Data Sharing Contract and Agreement will need to be signed by the researcher's institution and Bradford Teaching Hospitals NHS Foundation Trust.
