Description:
A Case Note Review completed by acute and community providers only, which reviewed consecutive deaths in the first two weeks of April 2019 and the first two weeks of May 2019 (acute providers) or deaths in April and May 2019 (community providers). Data collection period was June to October 2019 in England and Wales. Dataset focuses on patient demographics, recognition of death and the provision of an individualised end of life plan of care. Up to 40 cases were submitted per site in England and Wales.
