Symphony

A code signifying the hospital site where the patient attended the emergency care department.

Site

The date and time the patient arrived at the emergency care department.

Arrival Date/Time

The means by which the patient arrived at the emergency care department.

Arrival Mode Description

The name of the Health and Social Care Trust to which the emergency care department belongs.

Trust Name

The local district of the patient's usual residence.

Ward Of Residence

Current Postcode – used to derive appropriate aggregate geography. The postcode of usual address of the patient. Postcodes are alphanumeric references comprising of 2 halves.  The first half is an outward code of 2-4 characters and the second half is an inward code of 3 characters.  There is a space between each half.  The postcode is structured hierarchically, supporting 4 levels of geography unit: (1) postcode area - NI comprises 1 postcode area represented by the code BT, (2) postcode district - NI has 81 postcode districts, (3) postcode sector - NI has 314 postcode sectors with around 3000 addresses per sector and (4) postcode units - NI has over 60,000 postcode units comprising an average 20 addresses per unit.

The date when the patient left the emergency care department i.e. was either discharged or admitted to hospital.

The time when the patient left the emergency care department i.e. was either discharged or admitted to hospital.

A code signifying the type of attendance i.e. whether the attendance was a new, planned review or unplanned review attendance.

Attend Type

The review clinic within the emergency care department which was attended by the patient.

Review Clinic Description

A description of the incident that led to the attendance at an emergency care department.

Incident Type/ Attendance Reason

The care group allocated to the patient being treated.

Care Group

Area of the patient's body that the diagnosis relates to.

Diagnosis Site Description

The date when the patient arrived at the emergency care department.

Arrival Date

The time when the patient arrived at the emergency care department.

Arrival Time

A brief description of the reason for attending an emergency care department as recorded at Triage.

Triage Complaint

Indicates if the episode was a new attendance at an emergency care department.

Count of Attendances New

Indicates if the episode was a planned review attendances at the emergency care department.

Count of Attendances Planned Review

Indicates if the episode was a unplanned review attendances at the emergency care department.

Count of Attendances Unplanned

Indicates that the episode represented an attendance at an emergency care department (includes both new and review attendances).

Count of Attendances All

Indicates if the patient waited over 12 hours from the time they entered the emergency care department until the time they left i.e. were either admitted or discharged.

Total Waits - >12 Hrs

Indicates if the patient waited between 8 and 12 hours from the time they entered the emergency care department until the time they left i.e. were either admitted or discharged.

Total Waits - >8 - 12 Hrs

Indicates if the patient waited between 4 and 8 hours from the time they entered the emergency care department until the time they left i.e. were either admitted or discharged.

Total Waits - >4 - 8 Hrs

Indicates if the patient waited under 4 hours from the time they entered the emergency care department until the time they left i.e. were either admitted or discharged.

The date and time that the patient was triaged at the emergency care department.

Triage Pre Stream Datetime

The date and time that treatment of the complaint/ injury was completed.

Treatment Complete Datetime

The date when the decision to admit the patient to hospital was made.

Admit Decision Date

The time when the decision to admit the patient to hospital was made.

Admit Decision Time

Indicates the hospital or ward the patient was transferred to.

Transfer Hospital Description

The date when the patient entered the emergency care department (recorded at registration, triage, or 15 mins after the recorded ambulance arrival time, whichever is earlier).

Clock Start Date

The date and time when the patient entered the emergency care department (recorded at registration, triage, or 15 mins after the recorded ambulance arrival time, whichever is earlier).

The time when the patient entered the emergency care department (recorded at registration, triage, or 15 mins after the recorded ambulance arrival time, whichever is earlier).

Details the type of vehicle requested to transport the patient to a destination after they leave the emergency care department.

The date that vehicle transport was requested to convey the patient from the emergency care department.

Transport Request Date

The date and time when vehicle transport was requested to convey the patient from the emergency care department.

The time vehicle transport was requested to convey the patient from the emergency care department.

Transport Request Time

The last code assigned to the patient at triage signifying the priority of their condition.  The values in this field have a corresponding description in the "Triage Category Description - Last" variable.

A description of the last category assigned to the patient at triage to signify the priority of their condition.  The values in this field have a corresponding numeric code in the "Triage Category Code - Last" variable.
