OMICS-AD is an observational, longitudinal, multi-centric cohort study on cognitive impairment and Alzheimer"s disease (AD) in older people. This study aims to apply an integrative multi-omics approach to investigate cerebral and systemic biological pathway alterations with clinical relevance in preclinical and early clinical AD. Main objectives: 1. to investigate in depth the systemic and CNS molecular AD endophenotypes by integrative multi-omics. 2. to relate the identified molecular and pathway alterations to core AD pathology, clinical manifestation, and disease progression. 3. to derive and validate novel multimodal blood-based biomarker signatures. The study cohort includes cognitively healthy controls and participants with impaired cognition (MCI and mild dementia) with comprehensive clinical and neuropsychological assessment, blood and CSF collection, sMRI, and longitudinal follow-up at 18 and 36 months from baseline.
