Description:
This dataset is essential for population health research, service planning, and evaluation of primary care interventions. It enables linkage to other regional datasets (e.g. secondary care, ambulance, community services) to support end-to-end pathway analysis. All data are pseudonymised before being made available within the Secure Data Environment and exclude directly identifiable information.

Pathway:
The patient pathway in primary care begins with registration at a GP practice, where core demographic information and any known clinical history are recorded. Patients may access services through in-person appointments, telephone consultations, or online systems. During each contact, GPs and other practice staff record coded entries for symptoms, diagnoses, prescriptions, referrals, and results. Patients may be referred to secondary care, community services, or social care, with relevant details recorded in the GP system. Ongoing care includes regular reviews, medication management, vaccinations, screening, and long-term condition monitoring. GP data therefore provides a continuous, foundational record of each patient’s interaction with the NHS.
